【現病歴】症状・状態の経過（いつから、どのように）
【家族歴】ご家族・ご親戚の病気、問題など
【既往歴】これまでの心や体の病気
【生活歴】生まれ育ち、家庭、教育、職業など
などをうかがいます（申込・問診票をご参照ください）。

そして【診断】（複雑な際は暫定的）のもと【治療方針】をご提案いたします。

We will ask “Chief Compliant”, “History of present illness”, “Family history”, “Medical history” and “Life history”.
We will “Diagnose” and suggest “Treatment plan”.

“Bio-Psycho-Social model” （生物・心理・社会学的なモデル）
（薬物・心理療法・環境調整など）の治療に基づきます。

薬物は向精神薬（抗精神病薬、抗うつ薬、抗不安薬、睡眠薬など）です。
副作用や依存性など考慮して「必要最少限」となるよう、
ご相談の上、処方いたします。

適正な薬物療法は、”Cost Benefit” 費用（経済・時間）対 効果において、
推奨される治療方法です。

心理療法は以下1.2.3.4.5.の流れに沿います。
ご自身で可能な際は、医師の助言が中心となります。
お一人で困難な際は、医師の判断のもと、臨床心理士をご紹介いたします。
ただし、保険診療外として、カウンセリング料金が発生いたします。

1. 毎日の生活や行動を振り返ります（セルフ・モニタリング）。
2. ストレスに対する対処・行動を確認します（ストレス・コーピング）。
3. 現実的・効果的な解決方法を検討します（問題解決療法）。
4. 認知（考え方）や行動を修正します（認知行動療法）。
5. 価値観や人生観なども見直し、健康で幸福な人生を考えましょう。

必要に応じて血液・画像（ＭＲＩ）検査などをご案内いたします。

This treatment is based on “Bio-Psycho-Social model” (medication, counseling, environmental coordination).

Psychotherapy is carried out in the following order under the instruction of psychiatrist.

1. Review one`s lifestyle and behavior (self monitoring).
2. Review one`s ability to cope (behavior) with stress.
3. We will seek a realistic and precise solution for you.
4. A review will be done on one`s thought process (cognitive) behavior.
5. Finally we will discover healthy and happy view of life!

Certain cases may require blood testing, diagnostic imaging, or other treatments.

原則として健康保険による診療を行っております。
3割負担の場合、初診：約2500円、再診：約1500円です。
検査・薬剤・診断書・紹介状など別途料金が生じます。

自立支援医療（旧：通院医療費公費負担制度）も承ります。
健康保険を用いない診療（いわゆる自費診療）も可能です。

As a principle, medical insurance can be used for treatments.

For initial visits, costs will be borne by the patient will be approximately 2,500 yen.
Thereafter, the patient cost will be approximately 1,500 yen.

There will be separate charges for examination medication.
Additional charge for intensive counseling.

For certain cases, national medical aid can be used.
Treatments are available without health insurance.
